The Learning Brief for Safeguarding Adult Review Oliver has now been published and is available to read on the Learning Briefs page of the website.
Oliver is an 18-year-old male. His early childhood was impacted by domestic abuse and multiple harms, and he became a child looked after (CLA), subject to a care order and had several foster placements. This childhood trauma contributed to Oliver finding relationships difficult and engaging in behaviours that created risk such as self-harm and substance misuse. At the age of 17 he was placed under the Mental Health Act (MHA) in a Tier 4 Child and Adolescent Mental Health Services inpatient psychiatric intensive care unit, he was then moved to a general assessment unit while still under Section 3 of the Mental Health Act. Oliver then turned 18 and was moved to an adult inpatient unit. The move to this unit caused Oliver’s self-harm behaviours to escalate, and he suffered serious injuries as a result.
